Once you have booked your initial consultation you will be sent a confirmation email with a link to our New Patient form. Please complete and submit the form electronically and return to us. You will not be required to complete any paperwork if you are booking follow up appointments.
NB: If you are booking an appointment on behalf of another patient eg a child, please make sure the booking is made in their name rather than yours. If you cannot do this please add a note to the booking alerting us to the the name of the patient so we can alter it to the correct name.
If you have had a positive Covid test, please call us to rearrange your appointment and also do call if you are shielding or living with someone who is shielding.
Please continue to wear a mask in clinic.
Online payment is taken at the time of booking and your account credited with the amount you have paid.
Your appointment is NOT secured until you get an email confirmation with the date and time of your appointment.
If you do not receive this confirmation then you doNOT have a valid booking and we would ask you to call our Reception team on 01483 527945 who will be happy to help you.
